Connections For Hope Partnership: 2014 Impact Grantee

The goal of the Giving Circle of HOPE’s first Impact Grant was to celebrate our 10 years of giving by awarding our first large ($25,000) and transformational grant that would allow an organization to increase capacity or services in a major way.

An informal needs assessment of the community resulted in three major areas of need: transportation for those aging in place, needs of young adults with disabilities, and efficient and effective services to help the low-income, disadvantaged population.

Several local organizations that work to meet those needs where invited to apply. At the end of the selection process, members of the Impact Circle selected Connections for Hope Partnership as the winner.

With our Impact Grant, the Connections for Hope Partnership was able to expand the capacity of their community case management function, which is foundational to their integrated services model. This staff position supervises case managers and ensures that the clients can access and move through multiple services with accurate information, assistance and follow up. The grant increased the capacity of this position to serve an additional 10-12 clients per quarter.

Helping 40-48 families avert a crisis that can affect the wellbeing of children and prevent families from homelessness, a medical crisis or hunger was just the kind of impact that Impact Circle members were hoping to achieve!
