Outcome of the 2014 Impact Grant Awards

In 2014, The Giving Circle of HOPE granted $28,000 in Impact Grant funds to three nonprofits.


Grantee: Connections for HOPE Partnership with Cornerstones
Project: Build Community Case Management
Amount Awarded: $25,000
Purpose: Hire a full-time Community Case Manager (CCM) to take more referrals from the schools and housing prevention cases where people are at risk of homelessness. These were being referred to other parts of the County due to lack of capacity here in North County.
Outcome: checkmark.Having an additional CCM has allowed Connections for Hope to take more prevention community case management cases and access more housing assistance funds. During this year, C4HP and Cornerstones have been able to launch an Employment Pilot that is geared toward “closing the skills gaps” in growth sectors of IT, Healthcare and Accounting/finance. This innovative pilot program includes community case management for the participants to address many of the barriers to successful employment. The Giving Circle grant covered all of of the CCM’s time over the last 6-month period of the fiscal year. She served a total of 42 cases during this period.
